What Your DVA Card Entitles You To

If you hold a Gold or White Card, you can access physiotherapy, chiropractic and podiatry treatment through DVA, provided you have a referral from your GP or specialist — known as a D904 referral. We bill DVA directly for any treatment provided under DVA arrangements, so there’s no gap fee for you to find.
How long your referral lasts depends on your card and circumstances:

  • If you hold a Gold Card and have a Totally and Permanently Incapacitated (TPI) classification, your referral is valid for 12 months and covers unlimited treatment sessions in that time.
  • If you hold a Gold or White Card without a TPI classification, your referral covers 12 treatment sessions, after which your GP will need to issue a new one.

Your PEAK practitioner will keep your referring doctor updated as treatment progresses, so there’s no paperwork for you to chase. DVA can also help with the cost of getting to and from your appointments — worth raising with your practitioner if travel is a factor.

A Coordinated Team, Not a Single Practitioner

PEAK was founded in 2012 by Matthew Stewart, a physiotherapist and chiropractor, on a simple observation: care works better when the people treating you actually talk to each other. That’s still how we operate. Our physiotherapists, chiropractors and podiatrists work as one team rather than referring you out to separate, disconnected providers — which matters most when you’re managing more than one issue at a time, a reality for many of the veterans we see.

In practice, that means your physiotherapist, chiropractor or podiatrist will build a plan around what matters most to you, communicate with your referring GP, and bring in another team member if your treatment calls for it — without you needing to start the conversation over from scratch.

1/15 Lamington Street, New Farm. Open Monday to Thursday with extended hours, and Friday–Saturday mornings. Convenient for veterans in New Farm, Newstead and surrounding suburbs.

Getting Started Is Simple

  • See your GP or specialist and ask for a referral (D904) for physiotherapy, chiropractic or podiatry.
  • Book an appointment at PEAK Hawthorne or PEAK New Farm — whichever is more convenient for you.
  • Bring your DVA card and referral to your first visit. We’ll handle the DVA billing and keep your GP updated as treatment progresses.

Common Questions From DVA Patients

Do I need a referral every time I want to start treatment?

Yes — a referral from your GP or specialist (a D904) is required before treatment begins. Depending on your card and TPI status, that referral then covers either 12 sessions or, for TPI Gold Card holders, up to 12 months of unlimited sessions.

Will I have to pay anything out of pocket? 

No. We bill DVA directly for treatment provided under DVA arrangements, so there’s no gap fee for Gold or White Card holders receiving treatment for an eligible condition.

What if I don’t have a referral yet? 

Speak with your GP or specialist first. Once you have a D904 referral, you can book straight in with us at either clinic.

Can DVA help with getting to appointments? 

DVA can assist with certain travel expenses for treatment. Ask your practitioner or your GP for guidance on what applies to your situation.

Do I need to start with physiotherapy, or can I see a chiropractor or podiatrist directly?

Your referral specifies which service it covers, so it depends on what your GP or specialist refers you for. If you’re not sure which practitioner is the right fit for your condition, mention this when you book — our team can help point you in the right direction before your first appointment.
